How we learn about patient safety issues

We learn about patient safety issues in a number of different ways. Some examples include:

  • Information from patients and members of the public
  • Information from healthcare staff and organisations
  • Patient safety data
  • Healthcare publications
  • Coroner's reports

Hearing from a breadth of stakeholders and understanding experiences of people who directly interact with healthcare helps us to ensure that the issues we prioritise for investigation have a meaningful impact on patient outcomes.
